Summer Library Program Testimonial Examples
Testimonials are outcomes-based evidence, which can be statistical or anecdotal statements that demonstrate the value of the Summer Library Program to specific children, to schoolwork, or to the benefit of the local community. A testimonial is not a list of the activities or library events during Summer Library Program and is not provided by the librarian.

Summer Library Program Evaluation
The Summer Library Program Evaluation form is requested across the state. The information collected from the Summer Library Program Evaluation could impact future funding of the summer program from the State Library/LSTA grant. Please complete and submit your evaluation by Friday, September 2, 2016 so your statistics can be included in the State Library report.
The first portion of the evaluation contains questions regarding the Collaborative Summer Library Program (CSLP) and information requested from CKLS. The second part of the evaluation is where you will submit testimonials. Please complete and submit the entire evaluation. Failure to submit your evaluation by the September 2nd deadline may result in your CKLS Summer Library Program Incentive Grant being reduced (See the CKLS 2016 System Plan, page 9).
Testimonials are an important part of your Summer Library Program Evaluation and must be submitted to be fully eligible for the entire $200 of the Summer Library Program System Incentive Grant.

Summer Library Program Online Evaluation Form You are not able to save and come back to the form or to print it out- it must be completed and submitted in one sitting. After you complete and submit the Online Evaluation Form you will receive an email with a printable pdf version of the information you entered in the Online Form. You may print this pdf for your paper records. You will also receive a personalized email confirming your entries. Please note that numerical entries need to be rounded up to the next whole number and should not include dollar symbols or decimal points. Dollar amounts should be entered as 42, not $42.17. You must also enter an answer for every question, even if it does not apply to your library.

Young Adult Summer Library Program
The Young Adult Summer Library Program is for school ages grade 6 through high school. The program is developed to offer constructive incentives for teens to visit the library and to read.
The emphasis on reading is for the pure joy of reading and not for worrying about reading grade levels and book reports. By offering the summer program in the public libraries, the teens will have available to them a wide range of materials. We may never form an avid reader from among all the teens but they will possess a degree of library literacy and the importance of libraries in their communities for the rest of their lives.
The philosophy of the System is to develop a fun, informative, non-competitive theme related summer library program. The reluctant, or poor reader will consider a competative program as an overwhelming hurdle and won't even come into the library to sign up for the program.
